The company provides long term disability (LTD) insurance to replace a portion of your income in the event you’re unable to work for an extended period of Social Security Normal Retirement Age or 3.5 years, whichever is longest. At age 62 (and older), the benefit period will be based on a reduced duration schedule

What is Evidence of Insurability?

EOI is information about your health, which the insurance company may require to approve you for coverage. If you’re newly eligible and have not previously waived coverage, you can elect up to the guaranteed issue amount without submitting EOI. You may be required to submit EOI if you have previously waived this coverage or if you elect above the guaranteed issue amount.
